Vulnerable Elderly Patient Care Coordinator

We are sad to say goodbye to Indra, Our care Coordinator for Vulnerable Elderly patients and their carers. All at teh practice and her patients have expressed how much they will miss her. We would like to wish her all the best in her new endeavours as a social worker in the community.

We are delighted however to have now appointed two care coordinators

Mandy (Monday to Thursday)

Maneesha (Wednesday and Friday)

Our dedicated practice based Elderly patient care coordinators, provide a unique service to Watford Health Centre. They work closely with the GPs and the rest of our expanding multidisciplinary team at the surgery and coordinates patients care rapidly to the most appropriate health professional or team.

They are often the

First point of contact for those that are frail, living alone or housebound and for their carers, providing continuity

Provide links to community services including district nurses, OT and physio as well as mental health, social care

Provide post hospital discharge support working closely with our in house pharmacy team

Maneesha and Mandy help keep our growing elderly population as independent as possible, supporting them when things get too much, whilst helping with more rapid access to services to meet their needs.
