Requesting copies of your medical records
Patients must put their request in writing, either by email to watfordhealthcentre@nhs.net or by letter. In the request, patients must confirm which part of the record is required:
- Electronic only
- Electronic and paper notes
- Specific part, in which case precise details e.g. date range, condition, etc. need to be stated
The practice requires 28 working days to supply the records. Once completed, the practice will either send the records via email or advise patients to collect them from the surgery.
